Single Care

For those who prefer to pay by the visit, our Single Care Option is a single episode of care, whether that is an office visit or physical. It is a problem focused and episodic visit. Since there are no extensions of refills, Single Care patients are asked to keep track of their medications and call for an appointment two (2) weeks prior to the end of the prescription.

Due to the intermittent nature of this service, prescriptions are written for 90 days. Similarly each episode of illness requires an office visit. We do not treat work or accident related injuries, nor do we have expertise in handling chronic pain or adult ADHD with stimulants.

Enrollment

The enrollment option provides a closer relationship with Dr. Rendell. It includes as many office visits as necessary in a month to address your prevention or health issues, one complete physical exam per year, the availability of telephone consultation access to the physician, and medical form completion. Because of the more frequent visits for monitoring and health counseling with the physician, prescription refills can be authorized by having the pharmacy fax us a request. The enrollment fees are structured by age and are payable monthly, quarterly, semi-annually and annually.
